Do I Qualify for a Medical Marijuana Card (MMJ) in Florida?
  • You must be 18 years of age or older (<18 must be certified by 2 doctors)

  • You must be diagnosed by a certified physician with a qualifying condition

  • You must be evaluated in person for the medical card by a doctor certified to recommend medical marijuana in the state of Florida

  • You must be a permanent or seasonal FL resident (live in Florida at least 31 days a year)

  • Residency requirements: Permanent- Florida driver's License or State ID; Seasonal- 2 of the following:

    • A copy of a house deed or lease agreement

    • Utility bill no more than 2 months old

    • Mail from financial institution (<2 months old)

    • Mail from gov't agency (<2 months old)

Medical Records
  • For new patients, medical records documenting your qualifying condition are REQUIRED.

  • Before your appointment, the records must be reviewed by our physician.

  • Medical Records must not be more than 5 years old.

  • Cancer

  • Epilepsy

  • Glaucoma

  • HIV

  • AIDS

  • Post-traumatic stress disorder (PTSD)

  • Amyotrophic lateral sclerosis (ALS)

  • Crohn’s disease

  • Parkinson’s disease

  • Multiple sclerosis (MS)

  • Medical conditions of the same kind or class as comparable to the others listed (this may include Anxiety, Depression, ADHD, Hepatitis C, Lupus, Arthritis, Fibromyalgia, Chronic pain and other chronic conditions- contact us if you have questions about this.)

  • A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification.

  • Chronic nonmalignant pain caused by a qualifying medical condition.
